By reading it , I understood that the insurance company must be based in Indonesia .

I recently did read at Bali expats | Facebook people recommending Jagawisata (https://www.jagawisata.com/COVID19) , an Indonesian based company which has a specific insurance to cover this requirement @ Rp500'000 for the first 30 days after arriving in Indonesia .

From Jagawisata's website : '...Indonesia Travel Covid is a joint Insurance product of the Indonesian Insurance consortium in collaboration with the Ministry of Tourism..."

I believe you got it from here
Latest FCDO travel advice for Indonesia including on entry requirements, safety and security and local laws and customs.

The official one is from Indonesian Government, here
          https://covid19.go.id/artikel/2022/05/18/surat-edaran-kasatgas-nomor-19-tahun-2022
In Indonesian but you could use Google translator to translate it. p7, letter f.
          "For foreign travellers, attach proof of ownership of health insurance" which includes financing for the handling of COVID-19 and medical evacuation to a referral hospital with a minimum amount of coverage according to determined by the organizer, manager, or local government local;
It does not say Spell out Quarantine Period requirement. Even it was true, common sense dictate if you already have enough fund ready during your stay which include accommodation, subsistence. This cost might be similar or higher than quarantine hotels.

The COVID-19 Insurance does not need to be Indonesian Insurer. (e.g it does not need to be Jagawisata) Any International Travel Insurance with additional COVID-19 coverage will meet that requirement. If you are traveling fome the UK any travel Insurance such as Admiral, Staysure, trail finders, Coop, etc will meet that requirement. This has been confirmed by my colleague who have done the whole process and now returned to t he UK.

Just in case someone doubt that The travel Insurance does not have to be "Indonesian Insurer (Indonesian legal entity)
" here is also confirmation from a person who asked Indonesian Embassy In London and posted it on Tripadvisor
            https://www.tripadvisor.co.uk/ShowTo...tory-Bali.html

            15. Re: Insurance with an Indonesien legal entity now mandatory?
            12 May 2022, 11:08
"I just wrote the following message to Indonesian Embassy in London, "Hello, I am travelling to Bali on 22/05/22. According to the FCO website a condition for entry is that I should, "show proof of full health insurance from an insurance company which is incorporated as a legal entity in Indonesia. Coverage should include COVID-19 treatment". I have worldwide travel insurance which has Covid 19 cover and also covers me for travel to Indonesia. My insurance is underwritten by Zurich Insurance Plc; however I have no idea whether this company is incorporated as a legal entity in Indonesia or not. Would my insurance policy be adequate to enter Bali?"
Within moments I received this reply, "Dear Sir/ Madam
You may purchase an insurance from an international companies or through and indonesian provider through website https://www.jagawisata.com/
The insurance must cover all medical expenses and evacuation, as well as covid-19, during the entire stay.
            Best regards,

Adindas , to me your post no.5 above was enough . But anyway at the end I would still recommend Jagawisata because it is a insurance specific to cover the Indonesian Government's requiremnt and I guess the price is relatively low .
