I had my 4D Ultrasound done a couple months ago at YPK Mandiri Hospital in Menteng. The phone number is 021-3909725. The Ultrasound doctor was Dr. Bambang Karsono, SpOG. I had to book one month in advance, because he was very busy. My Obgyn suggested it at 6 months, and gave me a referral letter. Cost was 670,000 rupiah. The final results and pictures were available a week later.
